NOTE: Make sure to attach to this form the child’s colored birth certificate, colored mother’s/guardian’s ID card and father’s death certificate. Scan them in the order that the child's birth certificate is on page 1, father's death certificate on page 2, mother's ID ( death certificate if deceased) on page 3, guardian's ID on page 4 and any other supporting document to follow.

You are required to bring with you the following items upon reporting and will be checked before your child's admission is formalized.

1. Two (2) colored copies of child’s birth certificate.
2. Two (2) copies of parent’s/guardian’s ID card.
3. Two (2) passport-sized photographs (to be taken in school uniform or a kanzu/qamis).
4. One (1) full photograph (to be taken in school uniform or a kanzu/qamis).
5. School uniform;

• Two (2) navy-blue pair of trousers.
• Two (2) white shirts.
• One (1) navy-blue long-sleeved sweater.
• One (1) pair of black leather shoes.
• One (1) pair of navy blue track suit with two white stripes.
• Two (2) pairs of grey school socks.
• 3 pieces of navy blue kanzus (qamis).

NOTE: All uniform items MUST be labeled well with the pupil’s name.
NO CIVILIAN CLOTHES WILL BE ALLOWED

Other requirements.

• 500 Kenya shillings school registration fee
• 200 Kenya shillings interview fee
• 1,200 Kenya shillings for madrasa text books, report forms (Paid once per year)
• 1,500 Kenya shillings for computer studies.
• 250 Kenya shillings for assessment book (for grade 8 pupils and below)
• Enough exercise books, pens and pencils.
• School bag
• Other personal effects, e.g., toothbrush, toothpaste, hair comb, hair oil, jelly, shoe brush, shoeshine, soaps etc.
• One small metal box (sanduku) for keeping personal effects.
